IMPORTANT: mLegitymacja (virtual student ID card) is only available for students with PESEL number (more info about PESEL here). Below information concerns only issuing physical student ID card. Instructions concerning mLegitymacja can be found here. mLegitymacja provides the same discounts as the physical student ID card.
Physical card: Fill in the application through USOSweb (Student’s section -> Requests -> Application for an electronic student ID card), AFTER placing the fee.

Through your individual account number, which you can find in:
At Dean’s office of your faculty.
At the beginning of each semester, visit dean’s office of your faculty – they will extend validity by placing sticker on your student ID card.
50% discount for train and public transport tickets; it also may provide discounts for tickets in culture facilities such as museums, cinemas etc. and some gastronomic places – if you are not sure if discount is provided it is always worth to ask about it; often info about discount is placed in visible location inside the local.
Immediately inform Dean’s office of your faculty, they will block your lost card. Then place a fee of 33 zł on your individual account, fill in and submit “Declaration of loss of an electronic student identity card” through USOSweb. When ready, collect it at Dean’s office.
